Clinical Pharmacists work within your GP practice. They are different to the community Pharmacists that work in Pharmacy stores. Our pharmacists work to make sure all aspects of your medication is appropriate, safe and effective.
Our Pharmacists have many roles and check medication added from hospitals for example. They also do a regular review of our patient’s medication.
They are active in contacting our patients and discussing with you aspects of this. They are the best person to talk to, to review your medication and will have their own appointment list.
Our Clinical Pharmacists do a lot of work in the background to support your safe prescriptions. You may get a telephone review of your medicines, where a Clinical Pharmacist will ring you and discuss your medication with you.
They work in the practice with the rest of our team and will have access to your appropriate clinical records.
Care co-ordinators are key workers in the practice and are able to link in with groups of patients to make sure that care is happening and that nothing and no one is missed.
Certain groups of our patients have certain needs and our care co-ordinators will work to check this is done, they link in with the clinical teams to arrange set care. An example of this would be to ensure that all our care home residents are reviewed by the clinical team regularly and their medicines are reviewed.
Our care co-ordinators in our PCN are currently working with groups of patients including care home residents, those with multiple health needs, patients with cancer and patients with severe mental illness.
Our Health and well-being coach is highly trained in improving the lifestyle of our patients with proven interventions. We know that lifestyle changes and advice can help with the well-being of our community and help our patients regain control.
They offer a holistic review and will work with you to tailor and fit advice that you can find helpful and useful, hopefully leading to improved wellbeing. Following an assessment they will come up with a plan to help you improve lifestyle and hopefully your wellbeing.
You can ask to see our health and wellbeing coach via reception or you may be referred by reception or a practice clinician.
These are practitioners who are training for 2 years to become full nurse associates. They are required to gain experience in practice but also on placements.
Even as trainees they will have a range of skills and expertise. They can take blood, do dressings, injections, syringe ears and undertake health checks and screening.
When you have the above in your practice it may be the trainee nurse associate that will help you.
These are practitioners who have trained to be nurses for at least 2 years and they work alongside other Nurses and clinicians in a clinical team.
They will carry out a very similar role to traditional nurses in your practice or home and may include helping you with a wide range of problems including dressings, injections, minor illness, contraception and chronic diseases.
When you have an appointment with the nursing team you may well be seeing our nursing associates.
These are experienced practitioners who have previously had a lot of experience with an ambulance service and gained additional skills and experience working directly with GP practices. They help the practice with visits and offer appointments for certain problems.
They may visit you at home or you may have an appointment or review with them at your GP surgery.
A full, comprehensive assessment either at your home if needed or maybe the GP practice. They will diagnose, treat and link in with other members of the practice team or other services as needed.
Our physician associates work within our clinical teams and treat patients individually to a high standard. They are able to see our patients with acute and chronic illnesses. They have access to doctors and nurses as needed.
They will support you in a very similar way to a doctor or nurse and provide assessments, diagnosis and treatment for a wide range of conditions.
Our physician associates work in each practice and are independent practitioners. You can be offered an appointment in their clinics.
Our advanced PCN physiotherapists work directly with the practice and PCN to provide assessment and treatment for a range of conditions that can affect your functioning and mobility. They are able to diagnose and treat a wide range of conditions and are the right person to see first.
Our experienced physiotherapist will make an assessment of you and offer support, treatment and referral if needed.
You may have a telephone call or a face-to-face assessment with our physiotherapist after calling the practice. They will help and guide you about advice and you may need a follow up with them. They are able to prescribe and refer you to a more formal course of physiotherapy for more investigations if needed.
Each practice has a dedicated social prescriber who is able to take a holistic, complete view of our patient and help come up with options and solutions that will help. They link in with many support services that are available including mental health support, finance support and advice, community support groups and lifestyle advice.

You can access our social prescribing team via reception direct or you can be referred via anyone in your practice. A social prescriber will arrange to assess, with your permission and they will try and help you put together a plan to help your wellbeing.
General practice assistants (GPA’s) form part of a multi-disciplinary team within primary care, and provide a support role, carrying out administrative tasks, combined with basic clinical duties, helping to free up GPs time and contribute to the smooth running of appointments, improving patients experience in the surgery.
Pharmacy technicians play an important role within general practice and complement the more clinical work of clinical pharmacist, through utilisation of their technical skillset. Their deployment within primary care settings allows the application of their acquired pharmaceutical knowledge in tasks such as audits, discharge management, prescription issuing, and where appropriate, informing patients and other members of PCN workforce.
Performing general pharmaceutical duties under the control and supervision of a pharmacist or pharmacy technician. Undertaking a 2-year fixed term rotational training post and completing award to be able to register as a pharmacy technician with the GPhC.
The PCN manager works with clinical and operational teams within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource, and tools to deliver changes in a collaborative way. The PCN manager will support this vision to be developed in alignment with ICS strategy.
The PCN manager will plan, coordinate, and directly support delivery of transformation activity in general practice within a PCN – with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of general practice services. The post holder will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
The PCN manager will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.
The Primary Care Network Mental Health Team (PCN MH) is part of Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH). This service provides local specialist mental health support for adults aged over 18yrs. in Rotherham. It also supports GPs in the treatment of mental health issues in primary care. This service has several different pathways.
